New York State Traumatic Brain Injury Waiver Program Quality of Services Survey

As part of its ongoing efforts to advocate for services for individuals with brain injury in New York State, the Brain Injury Association of New York State (BIANYS) is conducting a short survey of individuals who receive services under the NYS Medicaid TBI Waiver program. The waiver provides services to New York residents who are eligible for Medicaid and meet the criterial for eligibility for nursing home services but are capable or receiving services within the community. 

Participants or family members can share their experiences about the quality of services under the TBI Waiver program by completing the survey online by clicking here.
